To register as a new patient with Dr. Lisa Schwartz, please follow these simple steps. You may want to print this page as a reference. Important: Patients who are transferring their care from University Health Service need to complete all of the New Patient Registration procedures, even if they have previously seen Dr. Schwartz at UHS.

1. Fill out the form at the bottom of this page, and Dr. Schwartz will send you a confirmation email within one week.
2. Thoroughly review the Office Policies page and contact Dr. Schwartz with any questions.
3. Your confirmation email will include a link to the online appointment schedule, and instructions on how to make an appointment online – it’s simple and only takes a minute! Find out how to make an appointment here >

The following steps must be completed prior to your first appointment:

1. If required by your insurance company, designate Dr. Schwartz as your Primary Care Provider. Call the customer service number on your insurance card to find out if this is required, and to designate Dr. Schwartz. (Dr. Schwartz accepts BlueCross/BlueShield, Excellus, MVP and Aetna insurance.)
2. Fill out all three forms on the New Patient Forms page:
• Patient Medical History: this is a secure online form, and only Dr. Schwartz can access your information.
• Privacy Acknowledgement: download, print, sign and bring this with you to your appointment.
• Information Release: download, print, sign, and send this form to your former provider to authorize them to release your medical records to Dr. Schwartz. UHS patients may make this request of UHS by phone at 585.275.2662.
  Important: Patients may want to inquire about fees associated with a record transfer. If costs are prohibitive, contact Dr. Schwartz to determine which elements of your record are essential. Records received by Dr. Schwartz can be returned to you after processing if desired.
