These three forms must be completed prior to your first appointment. Important: Patients who are transferring their care from University Health Service must complete these steps as part of their registration process.

• Patient Medical History: You may complete this secure form online by clicking here.
  
  
• Privacy Acknowledgement: To familiarize yourself with my privacy policy, please download the Privacy Acknowledgement form. Print, complete, sign and bring this form to your first appointment.
  
  
• Information Release: In order to transfer medical records from your previous doctor to me, please download the Information Release form. Print, complete, sign and send this form to your former provider (not to Dr. Schwartz) to authorize them to release your medical records to Dr. Schwartz. UHS patients may make this request of UHS by phone at 585.275.2662.
  
  Important: Patients may want to inquire about fees associated with a record transfer. If costs are prohibitive, contact Dr. Schwartz to determine which elements of your record are essential. Records received by Dr. Schwartz can be returned to you after processing if desired.
